Position Summary

This is an outstanding opportunity for a Nurse Practitioner with an interest in wound care who wants to grow clinically while building meaningful, long-term relationships with patients. TCP covers the full cost of wound care training and certification , allowing you to expand your skill set while stepping into a highly respected niche within senior care medicine.

You’ll work closely with physicians, nursing staff, and community leadership to deliver high-quality, on-site wound and primary care —all within a predictable, weekday rounding schedule.

What You’ll Do

• Provide hands-on wound care management in senior living communities
• Assess, treat, and monitor chronic and complex wounds
• Deliver primary and preventative care to a geriatric population
• Develop and adjust care plans focused on long-term outcomes
• Collaborate daily with physicians, nursing teams, and facility staff
• Document efficiently within an established rounding workflow

Qualifications

• Active Minnesota Nurse Practitioner license

• Reliable transportation required (rounding between communities)
