The Nurse Practitioner or Physician Assistant will serve as the lead member of the care team, working under the oversight of a supervising physician. They will be responsible for conducting home and telephonic visits, delivering holistic, high-quality care to members enrolled in managed care plans. This includes closing any identified open care gaps, Health Risk Social Needs gaps, and re-engaging members with primary care services and support to improve member quality of life and decrease total cost of care. The role will primarily take place in the home with supplemented telehealth visits to care for the complex medical population.
