Advanced Practice Provider (NP or PA) - Wellness Center

Physician Assistant or Nurse Practitioner Wellness opportunity 

Location: Victorville, California

Schedule: 5 days a week, 8-hour shifts

Job Description: An esteemed medical group is seeking a dedicated Advanced Practice Provider (NP or PA) for a prestigious Wellness Center located in Victorville. The successful candidate will be responsible for conducting medical annual wellness visits, focusing on hierarchical chronic condition codes. This role does not involve treatment; instead, it focuses on patient review, documentation, and ensuring compliance with Medicare guideline.

Key Responsibilities:

• Conducting medical annual wellness visits
• Utilizing hierarchical chronic conditions codes for accurate documentation
• Reviewing patient history thoroughly and documenting findings
• Facilitating referrals to primary care physicians as needed
• Ensuring compliance with Medicare guidelines for optimal reimbursement
• Managing a patient load of approximately 12 patients per day in 30-minute slots

Requirements:

• Minimum of 2 years of experience in primary care
• Valid Nurse Practitioner (NP) or Physician Assistant (PA) license
• Ability to travel to satellite offices near Victorville
• Strong understanding of Medicare guidelines and reimbursement procedures
• Excellent documentation skills and attention to detail

Compensation:

• Total compensation: $165,000 - $170,000 a year
• Additional incentives based on patient volume and assessment performance
