Glade Run's  Psychiatric Mobile Medication Program  serves Butler County adults 18 years or older with a psychiatric diagnosis, a history of inpatient hospitalizations, and difficulties in their progress towards recovery. The program provides the education, support, and skill development needed for individuals to become independent and responsible for their medications.

REQUIREMENTS:

Academic Requirements:

  1. Licensed Practical Nurse (LPN); Registered Nurse (RN) preferred. One year of psychiatric experience recommended. 

Emotional Requirements:

  1. Must have emotional stability to perform essential job functions.

Other Requirements:

  1. Must possess verbal and written communication skills.
  2. Valid driver’s license and acceptable driving record is required.
  3. Ability to drive for work purposes including to client homes and to job site. 
  4. Must be able to adjust to job requirements.
  5. Ability to implement agency policies and programming including but not limited to the following, Medication adherence, Medication administration, Medication reconciliation. 
  6. Must have acceptable PA State Police Clearance, PA Child Abuse History clearance, FBI clearance.
  7. Must be able to identify with the agency Mission Statement and support the agency’s Core Values.

CORE COMPETENCIES

  1. Collaborate with other service providers to ensure seamless transition of care from higher levels of care to the Psychiatric Mobile Medication program and at discharge from PMM.
  2. Collaborate with prescribers to obtain written scripts for individuals in the Psychiatric Mobile Medication program.
  3. Communicate any medication related side effects, refills and/or compliance issues with the Prescribing physician.
  4. Administer LAI injections for consumers with an active prescription in the Psychiatric Mobile Medication program.
  5. Collaborate to ensure any necessary lab work is obtained prior to the distribution or administration of medications.
  6. Collaborate with the pharmacy to obtain medications.
  7. Store and account for all medications in Glade Run’s possession.
  8. Collaborate with Psychiatric Mobile Medication Technicians to ensure proper disposal of unused medication. 
  9. Coordinate the delivery of medications to enrolled consumers.
  10. Educate consumers and staff members on the medications, side effects and target symptoms.
  11. Secure authorization for services.
  12. Ensure required documentation is completed per standards in the Electronic Medication Record.
  13. Ensure medication reconciliation occurs for any medications handled by Glade Run Lutheran Services.
  14. May be required to perform services in clients’ homes after required training and orientation is met.
